Eligibility Criteria
Inclusion
- Type 2 diabetes mellitus, hypertension and/or hyperlipidemia,
- On oral medication for two of the above conditions,
- Manage own medications,
- Must have access to a telephone,
- Not participating in other adherence education or counseling trials
