Inclusion
- Diabetes mellitus, type 2
- Obesity
- Ability to attend office visits in Manassas, Virginia
Exclusion
- Dementia
- Institutionalized living
- Retinopathy, neuropathy
- Pregnancy
- Serious health problems such as heart disease or stroke
