Eligibility Criteria

Inclusion

  • Children who are new patients
  • Receive a diagnosis of persistent asthma
  • Are in need of an asthma action plan

Exclusion

  • Diagnosed with intermittent asthma
  • An established patient who already has an asthma action plan
  • Not English or Spanish speaking
