Eligibility Criteria

Inclusion

  • member of participating primary care practice network
  • able to provide informed consent
  • able to complete data assessment tools in English

Exclusion

  • diagnosis of CHD, stroke, diabetes, breast cancer, ovarian cancer, or colorectal cancer
  • currently pregnant
  • evidence of cognitive impairment
