Eligibility Criteria
Inclusion
- Acute respiratory failure with mechanical ventilation
- Respiratory failure happened within 48 hours after admission
- Age \> 18 y/o
Exclusion
- Pregnanacy
- Transfer from other hospital with mechanical ventilation
- Mechanical ventilation after scheduled operation
