Eligibility Criteria
Inclusion
- have type 2 diabetes
- have a HbA1c equal to or \> 8.0%
- must speak English, Spanish, or Cantonese.
Exclusion
- Dementia
- Psychosis/Schizophrenia
- End Stage Renal Disease
