Eligibility Criteria
Inclusion
- age ≥ 65 years on January 1, 2000
- diagnosis of diabetes mellitus
- diabetes related visit to HTPN physician within the past year
- Resident of Texas
- Medicare insurance coverage
Exclusion
- Patient chart not available for abstraction
