Eligibility Criteria

Inclusion

  • age ≥ 65 years on January 1, 2000
  • diagnosis of diabetes mellitus
  • diabetes related visit to HTPN physician within the past year
  • Resident of Texas
  • Medicare insurance coverage

Exclusion

  • Patient chart not available for abstraction
