Eligibility Criteria
Inclusion
- Hypertensive individuals aged 40 years and above residing in selected clusters
- Individuals prescribed with hypertensive medication by their general practitioners in related cluster
Exclusion
- Pregnant women will be excluded
- Mentally unstable or bed bound patients
