- Inclusion Criteria:
- Diabetes
- Hypertension, as diagnosed by the average of two or more blood pressure measurements taken during two or more study visits following an initial screening
- Race is 'Black' or ethnicity is 'Hispanic'
- Patient at participating Community Health Center
