Eligibility Criteria
Inclusion
- participation in a Medicaid managed care plan
- resident of identified zip codes in Brooklyn, NY
- frequent hospitalizations and low frequency of primary doctor visits
Exclusion
- residents outside the catchment area
- patients not being managed in a Medicaid managed care plan
