Eligibility Criteria

Inclusion

  • age of 21 years or older,
  • a documented serum Troponin I level of greater than, or equal to the upper limits of normal in each hospital, and
  • a working diagnosis of ACS in the medical record.

Exclusion

  • inability to speak English or to complete the enrollment interview, and
  • discharge to a non-home setting.
