Eligibility Criteria
Inclusion
- age of 21 years or older,
- a documented serum Troponin I level of greater than, or equal to the upper limits of normal in each hospital, and
- a working diagnosis of ACS in the medical record.
Exclusion
- inability to speak English or to complete the enrollment interview, and
- discharge to a non-home setting.
