Inclusion
- EF \< 45 %
- NYHA 2 - 3
Exclusion
- Acute coronary syndrome within last 6 weeks.
- Pregnancy
- FEV1 \<1.0
- Cancer
- Any severe disease which could interfere with the treatment or the outcome
