Eligibility Criteria

Inclusion

  • Discharged from hospital with CHD
  • Age 75 years or less
  • Living in hospital catchment area
  • Able to understand Swedish
  • Willing to accept randomized group

Exclusion

  • Psychiatric disease
  • Participated in similar study previously
