Eligibility Criteria

Inclusion

  • COPD and/or HF diagnosis; confirmation based upon medical chart review.
  • clinically significant symptoms for anxiety and/or depression.

Exclusion

  • cognitive impairment
  • presence of bipolar, psychotic or substance abuse disorder.
