Eligibility Criteria
Inclusion
- Parents/guardians with legal custody of a minor child
- Parents of children 11-18 years of age.
Exclusion
- Parents under the age of 18
- Allergy to vaccinations
- Parent has made an informed decision not to vaccinate their child
