Eligibility Criteria
Inclusion
- Age 18 or older
- Proficient in English
- Chronic hemodialysis for minimum 90 days
- Does not currently have a living will
Exclusion
- Blindness
- Cognitive impairment that prohibits subject's provision of informed consent
