Eligibility Criteria
Inclusion
- At least one of the following:
- Cognitive impairment,
- UTI (urinary tract infection)
- Pneumonia,
- Status post hip fracture or trauma,
- \>5 medications,
- Previous hospitalization in past 6 months
Exclusion
- Refusal to participate
- Terminally ill
- On ventilator
- Delirium based on CAM-ICU
