Eligibility Criteria

Inclusion

  • adults
  • type 2 diabetes
  • taking medications for diabetes
  • medication non adherent

Exclusion

  • nursing home residence
  • plans to move away in the next year
  • advanced illnesses such as hemodialysis, cancer or dementia
