Inclusion

  • being age ≥50
  • having been diagnosed with at least one chronic disease\* (e.g., heart disease, diabetes, cancer)
  • having access to the Internet/e-mail
  • being able to use the Internet/e-mail independently
  • currently residing in the U.S.
  • being able to read/write English.
