Inclusion
- being age ≥50
- having been diagnosed with at least one chronic disease\* (e.g., heart disease, diabetes, cancer)
- having access to the Internet/e-mail
- being able to use the Internet/e-mail independently
- currently residing in the U.S.
- being able to read/write English.
