Eligibility Criteria
Inclusion
- eGFR \< 60 ml/min/1.73m2 for at least 1 month
- no prior chronic dialysis or transplantation
- Written Signed consent form
Exclusion
- Age \<18 yrs old,
- Pregnant female,
- patients who plan to move
- Unable to give inform consent,
- Decline to participate
