Eligibility Criteria
Inclusion
- Patients must be 18 years or older
- Must be able to read
- Must speak English
- Capable of providing informed consent
- Must be receiving continuity care from and enrolled clinician
Exclusion
- Currently hospitalized
- Awaiting heart transplant
