Eligibility Criteria
Inclusion
- Montreal Cognitive Assessment score 22 or greater
- consent with observed teach back confirmation
- discharge from sub-acute rehabilitation
Exclusion
- Montreal Cognitive Assessment score less than 22
- inability to "teach back" during consent process
- non english speaking
