Eligibility Criteria
Inclusion
- aged 18 years and older
- current smoker
- English speaking
- Patient at Mile Square Health Center (Main, Englewood, South Shore, Auburn Gresham, Cicero, Back of the Yards)
Exclusion
- aged less than 18
- non-smoker
- non English speaking
- Not a patient at MSHC
