Eligibility Criteria
Inclusion
- HbA1c greater than or equal to 7.7%
- from urban or rural area
- body mass index greater than or equal to 30
Exclusion
- gestational diabetes
- pregnancy or lactating
- not ambulatory
- intellectual impairment
- unmanaged comorbid psychiatric diagnosis (including eating disorders)
