Eligibility Criteria
Inclusion
- No prior diagnosis of dementia (defined from EHR diagnosis codes) and not receiving medications for dementia
- Active patient at participating clinic
- Adequate data to calculate eRADAR score
Exclusion
- \- Currently receiving hospice care
