Eligibility Criteria

Inclusion

  • Age \>18 years
  • New ischemic DFU \<6 months
  • English or Spanish speaking
  • Able to provide consent

Exclusion

  • incarceration or institutionalization
  • unwillingness to participate in the PAI program
