Eligibility Criteria
Inclusion
- • Adult participants enrolled in AMPATH's CDM Program with uncontrolled hypertension (SBP ≥ 140 or diastolic BP (DBP) ≥ 90)
Exclusion
- hypertensive emergency requiring immediate medical attention,
- terminal illness, and
- inability to provide informed consent.
