During the study, researchers will monitor participants' choices and use of home dialysis over one year. They will assess changes in patient activation and health-related social needs from baseline to six months and one year. Participants will be followed and evaluated through surveys and interviews to understand how the intervention affects their care experience. The study is sponsored by Montefiore Medical Center and will take place from December 2025 through May 2028.

Eligibility Criteria

Eligible

You may qualify if you...

  • Age 18 years or older
  • Advanced chronic kidney disease with an eGFR of 25 ml/min/1.73m2 or less
  • Able to speak English or Spanish
  • Provide informed consent
  • Followed by a nephrologist and seen within the last 12 months
  • Willing to receive interdisciplinary care including nurse practitioner-led CKD education and care coordination
Not Eligible

You will not qualify if you...

  • Active malignancy
  • Anticipated survival less than 1 year
  • Choosing medical management only (non-dialysis supportive care) for kidney failure
  • Planning to relocate outside New York City within the next 12 months
