The goal is to see if a fully digital workflow can standardize surgery, minimize bone removal, and improve healing results. Participants receive a personalized static surgical guide designed from high-definition CBCT and intraoral scans. On surgery day, local anesthesia is given, and the guide is fixed in place to direct a 4.5-mm trephine bur for a single-step bone and root-end removal. Ultrasonic tips prepare a root-end cavity, which is then filled with a bioceramic material. Postoperative medications are given uniformly. Follow-up includes suture removal at one week and clinical plus CBCT imaging evaluations at six months to assess bone healing and surgical precision. Participants will record pain and swelling levels on days 1, 3, and 7 after surgery using self-assessment forms. The study measures include the positional and angular accuracy of the surgery compared to the digital plan, pain and swelling scores, lesion volume changes, clinical healing status, and radiographic bone healing. The total participation lasts six months, with visits for follow-up imaging and clinical assessment to monitor recovery and treatment accuracy.
