Additional outcomes include documentation of resuscitation preferences and ACP in electronic health records. The study involves follow-up for up to one year to evaluate these outcomes and ensure comprehensive data collection on patient experiences and care choices.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Aged 21 or over
- Have a one-year prognosis or less as determined by Clover Health's validated algorithm
- Speak English or one of the 25 other languages available for the video intervention
You will not qualify if you...
- Visually impaired beyond 20/200 corrected
- Acute psychiatric illness or other comorbid condition that impairs cognitive ability to participate in advance care planning discussion as determined by the primary House Call Provider

Your Study Journey

Participants are screened for eligibility to participate in the trial.

Duration - Up to 1 year or until death
Participants continue to receive usual medical care at home. Those in the intervention group will have a visit with a trained clinician to view the Advance Care Planning video and discuss its content.
1 intervention visit and ongoing home-based care visits

Participants are observed for hospice utilization, advance care planning documentation, and patient-reported outcomes up to one year following the intervention or death.
