This research focuses on patients with severe emphysema undergoing bronchoscopic lung volume reduction treatment using one-way endobronchial valves (EBV). Emphysema often leads to peripheral muscle dysfunction, which affects exercise ability, quality of life, and survival. This is a single-arm study comparing muscle function before and after EBV treatment. During the study, participants will have muscle function measured through imaging and biopsies at baseline and again 6 months after EBV placement. The main outcomes include changes in quadriceps muscle phosphocreatine concentration and other biochemical markers of muscle energy use and structure. Safety and treatment effects will be monitored through these detailed assessments, with the total study period lasting about six months.

Eligibility Criteria
You may qualify if you...
- Patient is scheduled for a bronchoscopic lung volume treatment using one-way endobronchial valves
- Patient read, understood and signed the Informed Consent Form
You will not qualify if you...
- Unable to perform a cycle ergometry test
- Contraindications for magnetic resonance imaging (e.g., claustrophobia, implanted cardiac devices)
- Body length greater than 190 cm
- Currently on any anticoagulant therapy
- COPD exacerbation within 4 weeks prior to testing
