The aim of this study is to identify patients with DM at high risk of CVD using elevated N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels (\>125pg/mL), and (2) intensify therapy using renin-angiotensin system (RAS) antagonists, beta-blockers and sodium glucose co-transporter-2 inhibitors (SGLT2i) for primary prevention of cardiovascular events in this high-risk DM population.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Type 2 diabetes for at least six months
- Age 40 years or older, male or female
- No known cardiovascular disease (such as coronary stenosis > 70%, reduced heart function, history of heart attack, coronary procedures, heart failure hospitalization, stroke, limb amputation, or angioplasty)
- NT-proBNP level greater than 125 pg/mL
- Provided written informed consent
You will not qualify if you...
- Allergy or intolerance to study drugs or contraindications to them
- Already taking maximum doses of renin-angiotensin-aldosterone system inhibitors or beta-blockers
- History of diabetic ketoacidosis or type 1 diabetes
- Kidney function (eGFR) below 45 ml/min/1.73m2
- Symptomatic low blood pressure or systolic blood pressure below 100 mmHg at first visit
- Symptomatic slow heart rate, high-grade heart block, or heart rate below 60 bpm at first visit
- Diseases other than diabetes expected to limit life expectancy to less than two years
- Chronic infections, cancers, uncontrolled thyroid or liver disease
- Taking systemic corticosteroids
- Pregnant or breastfeeding women
- Any other condition that may affect safety during the trial as judged by the investigator
- Participation in another investigational drug trial
