Eligibility Criteria

Eligible

You may qualify if you...

  • Self-identify as African American or non-Hispanic Black
  • Screen positive for food insecurity in the past 12 months
  • Be age 40 years or older with at least one of the following: diabetes, hypertension, high cholesterol, overweight/obesity (BMI >25), or current smoker
  • Able to communicate in English
Not Eligible

You will not qualify if you...

  • Significant dementia indicated by confusion during interview
  • Participation in other cardiovascular disease clinical trials
  • Alcohol or drug abuse or dependency based on CAGE questionnaire screening
  • Active psychosis or acute mental disorder reported by participant
  • Life expectancy less than 12 months
