Who Can Participate
Eligibility Criteria
You may qualify if you...
- Self-identify as African American or non-Hispanic Black
- Screen positive for food insecurity in the past 12 months
- Be age 40 years or older with at least one of the following: diabetes, hypertension, high cholesterol, overweight/obesity (BMI >25), or current smoker
- Able to communicate in English
You will not qualify if you...
- Significant dementia indicated by confusion during interview
- Participation in other cardiovascular disease clinical trials
- Alcohol or drug abuse or dependency based on CAGE questionnaire screening
- Active psychosis or acute mental disorder reported by participant
- Life expectancy less than 12 months
