Eligibility Criteria

Eligible

You may qualify if you...

  • KPNC member, age >20 and age <45
  • New clinical diagnosis of type 2 diabetes within the prior 1-8 months
  • HbA1c 8% or higher (may be lowered to 7.5% if needed for recruitment)
  • Proficient in written and spoken English or Spanish
Not Eligible

You will not qualify if you...

  • Pregnant women
  • Type 1 Diabetes
