Who Can Participate
Eligibility Criteria
You may qualify if you...
- KPNC member, age >20 and age <45
- New clinical diagnosis of type 2 diabetes within the prior 1-8 months
- HbA1c 8% or higher (may be lowered to 7.5% if needed for recruitment)
- Proficient in written and spoken English or Spanish
You will not qualify if you...
- Pregnant women
- Type 1 Diabetes
