What this Trial Is About

Researchers are evaluating a program of care designed for patients with Chronic Obstructive Pulmonary Disease (COPD) who have recently been discharged from the hospital after an acute exacerbation. The study aims to assess how feasible and acceptable the program is for patients and healthcare providers, and to understand how much patients actually use the program over 90 days. The program combines established care practices from two institutions with new technologies to support patient health and reduce hospital readmissions. The program includes virtual pulmonary rehabilitation, integrated care, and remote clinical monitoring. Patients will use devices such as the Hyfe Smartwatch to monitor cough sounds, the FitBit Versa 2 to track vital signs like oxygen saturation and heart rate, and a home spirometer for lung function testing. They will also record audio sentences to analyze speech patterns related to respiratory health. This multimodal approach aims to provide personalized care and continuous monitoring after hospital discharge. Participants will be involved for 90 days during which their use of the program and feedback on its acceptability will be collected. Researchers will monitor feasibility through patient and clinician input and measure actual usage data from the devices and program activities. The study will help determine whether this care approach can be implemented on a larger scale and if it effectively supports patients in managing COPD after hospital discharge.
