Who Can Participate
Eligibility Criteria
You may qualify if you...
- 55 years or older
- At least 2 high blood pressure readings ≥ 130/80 mmHg within one month
- History of hypertension for more than 1 year
- Taking at least 1 antihypertensive medication
- Independent in Activities of Daily Living (ADL) and instrumental Activities of Daily Living (iADL)
- Own a smartphone
- Live within the Greater Houston Metropolitan Area
- Able to speak English
You will not qualify if you...
- Unable to understand or verbalize comprehension of the study (e.g., cognitive impairment or dementia)
- Unable to monitor blood pressure at home
- Plans to move outside Houston within the next year
