Participants will be assessed at the start and one week after the intervention ends, measuring frailty using the FRAIL Scale and physical performance with the Short Physical Performance Battery. Additional outcomes include falls, pain, mood, functional independence, nutrition, handgrip strength, comorbidities, and healthcare use. The study aims to support healthy aging through a practical program suitable for primary care settings.
