Evaluation of Premolar Autotransplantation to Replace Front Upper Teeth in Children Aged 7 to 15 Years Long-Term Study of Clinical, Radiographic, Functional, and Patient Outcomes

This research aims to assess the clinical, radiographic, functional, and patient-reported outcomes of premolar autotransplantation used to replace anterior maxillary teeth in young patients aged 7 to 15 years. It combines a retrospective cohort of patients treated between December 2019 and December 2025 with a prospective cohort enrolling patients until December 2028. The goal is to identify long-term success factors and prognostic indicators for this dental procedure in growing patients. Premolar autotransplantation is performed following a standardized clinical protocol involving digital planning with CBCT and STL models, 3D-printed donor tooth replicas, guided or conventional socket preparation, atraumatic donor extraction with extraoral time under 5 minutes, and semirigid splinting. Both cohorts follow identical surgical, orthodontic, radiographic, and follow-up protocols with clinical and radiographic evaluations up to 5 years. Participants are monitored at multiple time points including 1 week, 15 days, 1, 2, 3, 6, 9, and 12 months, then annually for up to 5 years. Assessments include tooth mobility, sensitivity, percussion sound, periodontal measurements, gingival recession, root development, pulpal healing, and radiographic signs of complications. Patient satisfaction, esthetics, function, and oral health-related quality of life are also evaluated using validated questionnaires. Data is analyzed to determine tooth survival, complications, orthodontic interactions, restorative needs, and overall outcomes.
