What this Trial Is About

Prostate cancer is the most common cancer among men in Denmark, often developing slowly and sometimes detected only after death from other causes. The prostate specific antigen (PSA) test can help detect prostate cancer but may lead to false positives, overdiagnosis, and overtreatment. National guidelines recommend PSA testing only for a small group of patients with specific symptoms and do not support routine screening. This trial evaluates a complex intervention designed to promote evidence-based use of PSA tests among general practitioners (GPs) in Denmark. The intervention involves a cluster package used in quality cluster meetings, where GPs typically gather to discuss clinical topics. This package includes a pre-recorded podcast featuring a urologist and GP to encourage reflection, a two-hour facilitated cluster meeting with slides, expert videos, clinic data, cases, and group discussions about PSA testing. Additionally, hand-out materials are provided to support further discussions within GP offices and improve communication with patients about PSA testing. The study compares quality clusters using this package with those not using it. Participants are general practitioner quality clusters in Denmark who agree to use the cluster package in a 2025 meeting focused on PSA testing. Researchers will measure changes in PSA testing rates over six months following the intervention. Data collection includes clinic data and feedback during meetings. The goal is to improve adherence to guidelines and reduce inappropriate PSA testing. The study excludes clusters that have already planned their 2025 meetings or participated in the pilot study.
