Eligibility Criteria

Eligible

You may qualify if you...

  • Must be over the age of 18 years
  • Have an Emergency Department triage systolic blood pressure greater than 180 and/or diastolic blood pressure greater than 110, with a repeat similar reading at least 30 minutes after triage
  • May have received treatment for elevated blood pressure but must have no evidence of end-organ damage such as acute stroke, acute coronary syndrome, acute kidney injury, or papilledema
  • May be admitted for deranged blood pressure only without complications or may be stable for discharge as determined by the treating emergency physician
  • Include referrals from the clinic to the Emergency Department with systolic and diastolic blood pressure values in the inclusion range
  • Can provide informed consent
  • Can receive phone calls and in-person visits; smartphone ownership is not required
Not Eligible

You will not qualify if you...

  • Pregnant patients
  • Terminally ill patients
  • Patients requiring hospital admission due to severe complications other than deranged blood pressure
