After six weeks, the RBPM group stops monitoring but receives weekly messages with heart health and infant care tips. Women will be involved from 28 weeks of pregnancy through six months postpartum. During the study, participants will measure their blood pressure regularly, receive coaching calls if in the enhanced group, and complete assessments of blood pressure control, weight, diet, physical activity, self-efficacy, and healthcare use. Nurses will provide reminders and referrals as needed. The main outcomes measured include blood pressure control rates and changes in blood pressure at six months, along with secondary outcomes such as weight, diet, physical activity, mental well-being, and hospital visits.

Eligibility Criteria
You may qualify if you...
- Pregnant women at least 28 weeks gestation
- Current hypertensive disorder of pregnancy or chronic hypertension
- Expecting a single baby (singleton pregnancy)
- Access to a phone
- Plan to deliver at Prisma Health-Midlands Hospital
You will not qualify if you...
- Multiple pregnancies (twins or more)
- Known fetal anomaly
- Do not speak English
- Already delivered a baby more than 14 days ago

Your Study Journey
Duration - 2 to 4 weeks
Participants are screened for eligibility to participate in the trial.

Duration - 6 weeks after delivery
Participants wear an automatic blood pressure cuff for 6 weeks after delivery. They measure their blood pressure daily for the first 2 weeks and 3-5 times per week during weeks 3 to 6. Nurses provide reminders and support as needed.

After 6 weeks, participants receive weekly messages about heart disease risk, healthy lifestyle tips, and infant development guidance.
Weekly messages
Duration - 10 weeks starting after delivery
Participants in the enhanced care group receive 10 weekly coaching calls from a community health worker focusing on heart-healthy lifestyle behaviors, including weight loss, nutrition, physical activity, stress management, and tobacco cessation. They also receive personalized weight tracking and social needs screening.

Duration - From 7 weeks to 6 months after delivery
For participants in the enhanced care group whose blood pressure remains high or who continue taking BP medications after 6 weeks, blood pressure monitoring continues three times per week until 6 months after delivery with ongoing nurse support.
