CommunityRx-Kidney Health A Technology-Driven Support Program for Managing Chronic Kidney Disease in Rural Eastern North Carolina

What this Trial Is About

Researchers are evaluating CommunityRx-Kidney Health CRx-K, a health information technology intervention aimed at improving chronic kidney disease CKD management in rural eastern North Carolina. CKD affects about one in seven adults in the U.S., and rural communities face challenges like limited medical access and resources, which worsen outcomes. This study focuses on enhancing care networks and well-being in these communities through integrated medical, social, and self-care supports. CRx-K includes three parts brief education on CKD needs, a personalized list of community resources called HealtheRx, and ongoing support from a clinic navigator for 12 months. The study is a pragmatic, randomized, single-blind trial in 35 rural primary care clinics, involving 634 adults with CKD. Participants are assigned to either receive CRx-K plus usual care or usual care alone. During the 12-month study, participants will be monitored for acute and ambulatory healthcare use, self-efficacy in finding resources, knowledge and use of social and self-care supports, unmet needs, and quality of life. Assessments occur at 6 and 12 months. The study team will track healthcare utilization and the impact of CRx-K on managing CKD within rural communities, with safety and outcomes followed throughout the trial.
