- 21 years old or older
- Referred by their healthcare provider via Ask-Advise-Connect
- Currently receiving care at one of the participating clinics
- Have daily access to their own smartphone (assistance provided if needed)
- No other household members already enrolled in the study

- Unstable medical or psychiatric illness
