Who Can Participate

Age: 18Years +
All Genders

Eligibility Criteria

Eligible

You may qualify if you...

  • Aged 65 years or older
  • Diagnosed with diabetes or hypertension plus at least one additional chronic condition such as arthritis, asthma, atrial fibrillation, Alzheimer's disease (not severe), cancer, chronic obstructive pulmonary disease, depression, heart disease, heart failure, hyperlipidemia, osteoporosis, kidney disease, or stroke
  • Had two or more clinic visits in the past 12 months
  • Plan to continue care with the enrolled clinician for at least 6 months
  • Do not have uncorrectable vision or hearing problems
  • Have not recorded a clinic visit for personal use in the past 6 months
Not Eligible

You will not qualify if you...

  • Unable to consent to participate
  • Diagnosed with schizophrenia, other psychotic disorders, or current substance-use disorders
  • Have uncorrected hearing or visual impairments
  • Living in skilled nursing homes or hospice care
  • Cognitive impairment indicated by a score of 3 or less on the six-item screener
  • Lack internet access
  • Do not speak English or Spanish
  • Do not have or are not interested in creating a personal email or shared email with a caregiver
  • Do not wish to create a patient portal account if they do not already have one
