Comparing Local Antibiotic Injection Versus Systemic Antibiotics with Ultrasound-Guided Needle Aspiration for Treating Breast Abscess in Females Aged 14 to 60

Breast abscess is a localized collection of pus that often develops after mastitis, especially in lactating women but also in prepubertal females. Its occurrence is higher in developing countries, reaching up to 16%. This condition usually causes painful, tender swelling of the breast and can result from factors like poor hygiene, milk stasis, and infections. Traditional treatment involves surgical incision and drainage with antibiotics, but this can cause scarring and require hospital stays. The study aims to compare systemic antibiotics against local antibiotic injections alongside ultrasound-guided needle aspiration to manage breast abscesses more effectively and reduce complications. The trial evaluates two approaches needle aspiration combined with systemic antibiotics and needle aspiration combined with local instillation of antibiotics. Needle aspiration is a minimally invasive procedure guided by ultrasound, avoiding hospital stays and general anesthesia. Local antibiotic instillation is being studied as an addition to aspiration to potentially reduce the number of procedures needed and lower failure rates compared to systemic antibiotics alone. The study involves random assignment to one of these treatment groups. Participants will be monitored from enrollment until nine days after treatment starts. Researchers will assess clinical symptoms like skin redness, pain, tenderness, and ultrasound evidence of pus to determine treatment effectiveness. The study also tracks whether local antibiotics reduce the need for repeated aspirations and complications. Safety and treatment success will be carefully observed during this period to better understand the benefits of local antibiotic use in breast abscess management.
