You may qualify if you...

  • Phantom teeth
  • Maxillary molars
  • Metal crowns made with conventional lost-wax technique for crown fabrication
Not Eligible

You will not qualify if you...

  • Overprepared or underprepared crowns
  • Defective metal crowns
  • Ruptured silicone films upon removal
  • Defective die stone casts
