- Seen in the Brigham and Women's Hospital Endocrinology, Cardiovascular Medicine, or Primary Care Clinics
- Dutch Lipid Clinic Network score of at least 3 points
You will not qualify if you...
- Already diagnosed with Familial Hypercholesterolemia documented in medical history, visit history, or problem list
