  • Seen in the Brigham and Women's Hospital Endocrinology, Cardiovascular Medicine, or Primary Care Clinics
  • Dutch Lipid Clinic Network score of at least 3 points
Not Eligible

You will not qualify if you...

  • Already diagnosed with Familial Hypercholesterolemia documented in medical history, visit history, or problem list
