Parents will also complete proxy questionnaires. The study will monitor program reach, fidelity, usability of the digital platform, and gather feedback from adolescents and healthcare providers. Data collection includes online questionnaires, interviews, and audio recordings of some digital meetings, with an emphasis on supporting adolescents in managing their health during this critical transition.

  • Adolescents aged 15 to 20 years
  • Literate and able to read Swedish
  • Diagnosed with asthma, allergy, or have undergone a solid organ transplantation
  • Parents willing to complete proxy questionnaires
  • Able to use digital healthcare platforms

  • Non-literate individuals
  • Unable to read or speak Swedish
  • Presence of cognitive impairment

Outpatient Digital Transition Program

Duration - Approximately 2.5 years (from age 16 to 18.5 years)

Participants take part in the digitally delivered STEPSTONES transition program to support their transition from pediatric to adult healthcare. The program includes visits with a transition coordinator, education, peer meetings, and development of a personalized transition plan, all conducted through a digital platform.

Multiple digital visits scheduled across five steps of the program

Follow-up Assessments

Duration - At baseline (age 16) and at endpoint (age 18.5)

Participants complete online questionnaires to assess patient empowerment, transition readiness, satisfaction with transition, and quality of life at the start and end of the program.
