Who Can Participate
Eligibility Criteria
You may qualify if you...
- Participants or their surrogates can agree to use Onward transportation service
- Have scheduled medical appointments at Onward partner practices
- Age 60 years or older
- Live in the community (not institutionalized)
- Include 60 participants with self-reported cognitive problems, mild cognitive impairment, subjective cognitive decline, or AD/ADRD diagnosis
- Reside and attend appointments in San Francisco, Alameda, Contra Costa, Marin, San Mateo, or Sonoma counties
You will not qualify if you...
- Currently enrolled in another transportation study
- Already using free or heavily subsidized transportation services
- Receiving comfort care only
- Not wheelchair bound; if using a wheelchair, it must be foldable and allow transfer
- Transient or lacking consistent long-term housing
- Reside outside the San Francisco Bay Area
