Evaluating the Addition of Dramatherapy to Adapted Physical Activity for Adults with Obesity in Medical and Bariatric Care Programs

Obesity is a major public health issue in France, especially in the Hauts-de-France region where over 20% of adults are affected. This trial evaluates the effects of adding dramatherapy to a standardized adapted physical activity APA program for adults with obesity enrolled in medical or bariatric care pathways. The study aims to improve emotional expression, body image, self-confidence, psychosocial well-being, and long-term adherence to care through this combination. Participants are randomized to receive either the standard APA program alone or APA combined with dramatherapy. The APA program involves 12 supervised sessions over 6 weeks, including cardiovascular exercises, muscle strengthening, and movement activities suited to patients with obesity. The dramatherapy intervention consists of six weekly group sessions with theatrical expression, role play, improvisation, breathing exercises, and body-oriented creative activities led by a qualified therapist. During the study, participants undergo assessments at baseline, at the end of the 6-week intervention, and at 6- and 12-month follow-ups. These evaluations include quality of life, eating behaviors, body image, self-efficacy, anxiety, depression, adherence to the APA program, and anthropometric measures such as body mass index and body composition. A qualitative component explores patient experiences using interviews and body mapping. The total participation time is about 12 months post-intervention.
